3. Home Visits Foundation staff visit the young person and family at home to understand needs, interests, and aspirations, and to determine suitability of placement. Foundation team, student, parents/guardians During consultation period

5. Placement in a Hub Upon acceptance, students are placed in a hub based on learning style and emotional development—not age or academic level. Hub staff, foundation leadership Immediately after acceptance
6. Personalised Induction Plan A tailored induction plan is created using information from the student’s EHCP (section F) and “student voice” work. Hub team, student, family Induction lasts from a few days to several months (minimum 4 weeks recommended)
7. Gradual Transition to Full-Time Provision Students progress at a comfortable pace; the goal is full-time on-site attendance. Transition is flexible to avoid overwhelming emotional reactions. Hub team, student Induction period varies; full-time attendance is the goal
8. Outreach Hub Support (if required) For complex cases where on-site provision is not immediately suitable, outreach work is provided in home or community settings. Support is reviewed after 3 months. Outreach hub professionals, student, family Minimum 3-month programme before review
